CASE MANAGER:  Early Intervention Specialist


Note: Bilingual Spanish/English skills are strongly preferred.

We have 1 Case Manager/Early Intervention Specialist positions available in our Santa Rosa office serving children 0 - 3 years old. 

NATURE OF WORK: 

Provides intake and ongoing assessments of infants and toddlers eligible for Early Intervention Services under California Early Start program mandates.  Assists eligible children and their families with the coordination of other resources, in development of an Individualized Family Service Plan an participates in periodic reviews of the IFSP every six months or more frequently if conditions warrant, and takes an active role in transition services for toddlers.  Provides service coordination to assist the families in accessing services necessary to enhance the development of the child and the capacity of the family to meet the needs of the child.  May participate in educational and prevention out-reach activities and serve as liaison to other community organizations that work with populations at high risk for having a developmental disability. 

MINIMUM EDUCATION AND EXPERIENCE: 

Must have a Master's Degree in Social Work or a related field from an accredited university, be a credentialed Speech Pathologist or Nurse with a PHN or PNP certification and three years of experience in working with children in early intervention agencies and resources. 

  OR 

A Bachelor's Degree and three years experience in a professional position at NBRC or another California Regional Center with child case carrying responsibility can be substituted for the above.

Knowledge of Spanish Language is desirable.  Minimum qualifications may be waived depending on the agency’s need for specialized skills (such as language abilities).
