How does a person apply for NBRC services?
Call or write and
say he or she wants to apply. A Client Services Assistant will
direct your call to the Assessment Counselor or Referral
Coordinator.
How long Will they have to wait for a response?
They will be seen
within 15 working days.
Do they have to fill out forms?
Yes. It is important
that we have a complete past record for the person applying for
services. We do not want to duplicate what has already been done.
What if the records do not include all of the information
that is needed to determine if an applicant is eligible for NBRC
services?
NBRC will arrange
for additional assessments if recommended by the Assessment Team. Most often these are
medical, social, developmental and psychological assessments.
Applicants and their
families may submit independent assessments for consideration by
NBRC.
Who pays for these assessments?
NBRC or the
applicant's health insurance. There is no cost to the applicant.
What happens after the records are collected and the
assessments are completed?
The applicant and
family will be asked to meet with an Assessment Team member to
review the diagnosis and, if eligible, plan for services. It is
at this time that the eligible applicant becomes a client of
NBRC.
Goals, objectives
and plans are written and become the initial Individual Program Plan (IPP).
How long does the person wait before there is a meeting to
develop the Individual Program
Plan?
A maximum of 45 days
for Infants and Toddlers (age 0-3) to develop an Individual
Family Services Plan.
For all others, a
maximum of 195 working days from the day of his or her first call
to NBRC.
