What are Teams?
As a person
served by North Bay Regional Center, you will hear the term
"Interdisciplinary Team" or "IDT." This is
the group that identifies needs, develops a plan to do something
about the needs, and then sees that the plan is carried out. Once
services are in place, the Team reviews them to see that they are
beneficial to the individual.
The Team includes,
at a minimum, the person who is a client of the regional center,
his or her parents if the individual served is under 18 years of
age, or his or her designated representative, and a Coordinator(CPC). Others may be
included in the Team depending on what decisions need to be made.
Representatives of providers of service to the client often
participate on the Team. Other regional center staff may
participate if needed or required by regional center or State of
California rules.
When are Team meetings held?
Some meetings are
required and other meetings are held when needed. Any member can
ask that the Team have a meeting.
Once a person is
determined to be eligible for regional center services, the Team
meets to agree upon a plan of services to meet the individual's
needs. This is written in the "Individual
Program Plan," or an "Individual Family Service
Plan" for 0-3 year olds, and each Team member gets a copy of
this plan.
At least once every
three years, around the time of the individual's birthday, the
Team meets to review the plan to make sure it still meets the
individual's needs. If it needs to be changed, the Team changes
it. This meeting is sometimes called the "IPP Review."
An IPP may be held more frequently if needed.
At least every 6
months an IFSP review is done for 0-3 year olds.
